Afghanistan is considered an EXTREME medical risk location. The standard
of public health care in Afghanistan does not meet international standards. During the political and military
turmoil of the last 40 years, some medical facilities have been damaged. Repairing these and maintaining
existing
facilities is an ongoing challenge.
More recently, and with the assistance of several NGO's and governmental groups,
there has been a focus on constructing and reconstructing medical facilities. However,
shortages of skilled labor and funds are hampering the process. International military
facilities may offer a higher standard of care, but are not usually available to travelers
unless authorized by a commanding officer. Moderate and serious medical conditions are
often recommended international evacuation, usually to the United Arab Emirates. In any
event, we strongly recommend contacting MedAire’s 24/7 Global Response Centre for proper
advice and coordination of care.

Given the volatile security environment and high levels of poverty,
there is a high probability of civil unrest. Afghanistan has historically
been divided along ethnic lines; regional factionalism and in-fighting among
the various tribal groups further exacerbate the situation. The security
forces respond forcefully to violent protests, including by firing live
ammunition. Political demonstrations are often a target for militant attacks,
as illustrated by an explosion in September 2019 during a campaign rally for
former president Ashraf Ghani (in office 2014-2021) in Parwan province. As
the Taliban consolidate their governance, there have been several localised
demonstrations against the group, in the capital Kabul and other urban
centres. These have been forcibly dispersed by Taliban personnel using
tear gas and live ammunition.

\n\n

Protests of varying scale and
intensity take place in response to alleged offences against Islam.
In October 2020, a series of demonstrations occurred over controversial
comments made by French President Emmanuel Macron over Islam and
caricatures of the Muslim prophet Mohamed. In September 2017, public
criticism erupted over leaflets that were distributed in Parwan by
NATO forces, which included an image deemed offensive to Islam.
The Taliban claimed responsibility on 6 September 2017 for a
suicide attack near the entrance of Bagram airfield that injured at
least three Resolute Support (RS) soldiers, reportedly in retaliation
for the leaflets.

\n\n

Large demonstrations periodically take place
in protest at the government's inability to thwart high-impact
attacks, such as a 31 May 2017 truck bomb that killed approximately
150 people. Following the attack, more than 1,000 people gathered
near Zanbaq circle in Kabul. Subsequent clashes between demonstrators
and the security forces resulted in the death of at least four
people.

\n\n

Members of Afghanistan's minority ethnic and
religious communities, including members of the Shia Hazara
community, periodically hold demonstrations against perceived
discrimination and targeting by the Taliban. In July 2016, a group
of Hazaras held a demonstration in Kabul against government plans
for a power project to bypass a predominantly Hazara province;
the event was attacked by the extremist Islamic State (IS) group
and 80 people were killed.
